How do I get appointed?

Go to the Contracting section, select which company you would like to get contracted with, print the contract off, complete it, and fax the contract, a copy of your license, E&O and voided check to (866) 409-2359.

Weekly webinars are archived and can be viewed 24×7. AMZ University also provides online training and facilitates private training.

Does AMZ have an outlet for Long Term Care?
